The International Association of Corporate Administrators made a minor revision to the form Financing Statement, the form Financing Statement Addendum and the form Financing Statement Amendment which now have a revision date of 5/22/02.

Previously, these forms had a box which requested the social security number or employer identification number of the debtor. The revised forms have replaced the request for a tax ID number with "see instructions". The instructions tell the user to add the tax ID number only if the UCC is being filed in North Dakota or South Dakota.

In addition, two new forms have been created for use if multiple debtors and/or secured parties are named. The UCC Financing Statement Additional Party form (Form #UCC1AP) is to be used with the National Financing Statement form and the UCC Financing Statement Amendment Additional Party form (Form #UCC3AP) is to be used with the National Financing Statement Amendment form.
